School Counseling Program Survey
All schools receiving Comprehensive School Counseling Program funds are required to complete a formal Systemic Assessment and Curriculum Delivery Survey every three years. For Elementary School Counseling Programs, it is recommended that these surveys be administered every 3 years in alignment with the secondary school counseling program's 6-year review cycle to support program alignment and data analysis across feeder schools. The Systemic Assessment is completed by school faculty, parents, and students. The parent and student survey should reflect a sampling of the population that is representative of the overall school demographics. Results from the Systemic Assessment offer a data-informed direction for the implementation of a comprehensive school counseling program.
Our schools use Utah School Counselors Survey to measure student mindsets and competencies as well as durable skills which are consistent with the school counseling program components. Results from this survey allow school counselors to address student needs and outcomes in the areas of academic learning, life/career goals, student wellness and provide feedback for CTE and Counseling Programs. School counselors utilize the information to deliver intentional classroom instruction, design effective individual Plan for College and Career Readiness meetings, and support the academic, career, social, and emotional development of each student.
What is the Intended Use of the Data Collected?
The results of the surveys are used to help our teachers, counselors, and administrators identify strengths and weaknesses, ensure appropriate student support, develop improvement plans and monitor progress. As with any organization, we continually strive to improve what we do with respect to supporting our students and their families.
Data Collection Information
Student personal information will be collected, used, shared, and retained only for purposes authorized by an educational institution/agency.
Utah School Counselors Survey implements technical, physical and administrative safeguards which are designed to reasonably protect the security, privacy, confidentiality, and integrity of student personal information against risks.
Utah School Counselors Survey will never sell student information.
Utah School Counselors Survey will not use or disclose student information collected through an educational/school service (whether personal information or otherwise) for targeting advertisements to students.
What persons or governmental entities will receive student-level data?
Identifiable student level data is considered an education record and, pursuant to Utah Code 63G-2-107, is governed by the Family Educational Rights and Privacy Act, 34 C.F.R. Part 99.
What persons or governmental entities will receive the data on a regular or contractual basis?
Other than the Utah State Board of Education, Weber School District does not have a contract with any person or governmental entity to receive the data on a regular ongoing basis.
What will my child do?
Your child will be asked to complete the online surveys during school hours. All surveys are reasonably short and are completely confidential. Students who have been excluded from participation in the surveys will be given an educationally relevant activity to engage in while the other students are completing the surveys.
